NICUにおける清潔ケア実施時の感染防止対策を明らかにする事を目的に新生児医療連絡会に登録されている全国のNICU223施設を対象とし, アンケート調査を実施し, 112施設から回答を得た. (回収率50.2%)清拭・沐浴時には全施設が手洗いを実施しており, 手洗いの方法については「石鹸や消毒薬を用いた手洗い実施後に擦式消毒薬を併用する」が66施設(86%)であった. 清拭時の手袋の着用について清拭時に手袋を着用している施設が89施設(79%)そのうち82施設(92%)が未滅菌手袋を使用していた. 保育器の清掃については, 「毎日実施する」が106施設(94%)で「消毒薬を用いて」73施設(65%)「未滅菌タオルで実施している」が38施設(35%)で方法として最も多かった. 保育器の交換頻度については「1週間に1回」が62施設(55%)と多く, 交換基準については内容が異なっており, 交換に対しての目的や留意点により, 各施設に違いが見られた. 日常ケアにおいても手洗いについては標準予防策に順じて実施している施設が多いが, その他のケアに関する基準や手順については違いがみられた. 新生児の特殊性をふまえた感染防止対策はエビデンスが明確にされていないものが多いが, 現状においては, 施設により異なる状況を認識し日々の清潔ケアにおける効果的な感染予防対策を検討する必要があると考える.

To clarify the infection prevention measure taken in NICU, we implemented a questionnaire on 223 NICU facilities which are registered in Japanese Neonatologist Association, and retrieved from 112 facilities (collection rate 50.2%). Hand washing was performed in all facilities when bathing or blanket bathing the patient. 66 facilities (86%) answered that their way of hand washing is "washing hands using soaps or sterilizing reagents followed by spray type disinfectant". On question about whether gloves are used during blanket bathing, 89 facilities (79%) answered yes, and 82 (92%) were using non-sterilized gloves. 106 facilities (94%) cleans incubator every day, where 73 (65%) using sterilizing reagents, 38 (35%) using non-sterilized towels which is the most common way of practice. About the rate of changing incubators, 62 facilities (55%) answered "once a week", and is the most obtained answer. Criterion on changing incubator differs by facilities in terms of the purpose and the points to keep attention. While hand washing is performed as standard precaution in most facilities, criteria and procedure concerning other hygiene care differed. Although many infection prevention measure considering neonatal specificity have unclear clinical evidence, under current situation, it is required to review the effective infection control measure in daily hygiene care with recognition of the facility difference.
