本研究は, Drotarらの仮説モデルを基盤にしながら, 先天奇形を持つ子どもを出産した親について, 出産したことに対する反応と出生した子どもに対する思いの2側面から記述することを目的とした.

データ収集方法は子どもの疾患別にグループを構成し, フォーカスグループインタビューを行った。対象は口唇口蓋裂グループが4名, 先天性四肢障害グループが7名, 二分脊椎症グループが6名, ダウン症候群グループが4名であった. 本研究におけるデータは, 逐語録として作成したインタビュー内容と母親らが描いた図とし, グループ別に, 探索的内容分析を行い, さらに4グループの親の反応の共通性と相違性を分析し, 先天奇形の特性を包括した親の反応について検討した.

その結果, 先天奇形を持つ子どもを出産したことに対する悲嘆反応がある中でも, 親は早期に子どもへの肯定的感情を持っていた. 出産したことに対する悲嘆反応が沈静しても, 〈適応〉から〈再起〉までの1年間は不安定であった. 先天奇形が持つ特性によって, 出産したことに対する反応および出生した子どもに対する思いは特異的であった. 〈再起〉とは, 障害受容ではなく, 不安や自責の持ちつつも, 障害を持つ子どもとの人生を積極的に生きようとする始まりとして位置づけられた.

Purpose of this study was to describe the reaction of parents to the birth, and to their infant with a congenital malformation.

This descriptive study was carried out on four groups: cleft lip and palate, hand and foot malformation, spina bifida, and trisomy2l. Data were collected via focus group interviews. Group members numbered from four to seven. Interviews were recorded on a tape recorder and decoded. Each group made a drawing of their reactions.

Subjects had a positive reaction and attachment to their baby, while at the same time they were crushed with grief after birth. After a period of intense emotional upset, emotional instability followed for a year. Each group showed a unique emotional condition. 'Reorganization' was the start of living with their child constructively with feeling of worry and guilt.
