IA-DSAを用いた脳循環時間測定による閉塞性脳血管障害の循環動態に関する臨床的研究第一編 : 測定理論と正常例における検討 <原著>

経動脈注入法によるdigital subtraction angiography(IA-DSA)を用いて脳循環時間(cerebral circulation time : CCT)を測定した。脳循環動態把握におけるCCTの臨床的有用性を検討する目的で, その基本となる正常例の検討を行った。正常例57例(男性31例, 女性26例, 平均年齢51.2歳)におけるCCTの平均値は3.87秒であった。左右差はなく, 再現性も良好で, 年齢的にも20～70歳の間では0.4秒以内の差であり, 極めて安定した脳循環の生理学的指標であることが判明した。ただし女性3.59±0.45秒に対して, 男性4.15±0.54秒と, 女性において有意にCCTが短いという性差が認められた(p<0.001 : unpaired t-test)。

The author estimated the cerebral circulation time (CCT) by using intra-arterial digital subtraction angiography (IA-DSA). The CCT was defined as the time interval of the time density curve between the arterial peak and the venous peak. Normal control values are necessary for routine clinical analysis of the CCT to evaluate cerebral hemodynamics. The author measured the CCT of 57 normal individuals, and the mean CCT of them was 3.87±0.57 seconds. There was no significant difference between right and left, and the reproducibility was good. The 70 decade of age persons showed later CCT of 0.4 seconds than the 20 decade of age persons, but there was no difference. Therefore, the CCT is considered as the extremely stable physiological parameter to analyze the cerebral circulation. However, the CCT of men was significantly longer (4.15±0.54 seconds) than that of women (3.59±0.45 seconds) (p<0.001 : unpaired t-test).
