切断肢再接着におけるフリーラジカル(O_2^-)の関与 : ラットの骨格筋の機能的, 組織学的評価 <原著>

Abstract
切断肢再接着時に発生するフリーラジカル(O_2^-)が骨格筋の機能回復に及ぼす影響について検討することを目的とした。まず, 阻血時間と阻血再灌流時のO_2^-発生量の関係を求めるため, ラットの切断肢を再接着し, 血流再開時に発生する活性酸素(O_2^-)をケミルミネッセンス法(CLN法)によって測定した。次に, 骨格筋の機能回復の評価のために再接着後3カ月で, 下腿三頭筋の筋収縮力と筋湿重量を測定し, 組織学的評価を加えた。また, O_2^-発生量との関係を統計学的に検討した。さらに, 切断肢再接着後の骨格筋組織の経時的な変化を組織学的に検討し, 以下のごとき結果および結論を得た。1)阻血再灌流時の, O_2^-発生量は, 6時間阻血群が3時間阻血群に比べて有意に増加した(p<0.05)。2)再接着後3ヵ月で, 筋収縮力および筋湿重量は, 6時間阻血群が3時間阻血群に比べて有意に低下した(p<0.01)。3)筋収縮力および筋湿重量とO_2^-発生量の間に負の相関を認めた(p<0.01)。4)組織学的所見では, 再灌流後1時間で, 6時間阻血群は3時間阻血群に比べ細胞間浮腫が増加し, 血管周囲の炎症細胞浸潤も増加した。また, 再接着後3ヵ月の時点で, 3時間阻血群はほぼ正常な回復を認めたが, 6時間阻血群では筋組織の障害が高度で回復は不良であった。5)阻血後再灌流時のO_2^-発生量は, 再接着肢の骨格筋機能回復の程度を予知できる臨床的に有用な一つの指標となりうる。
Abstract
The relationship between ischemic time and production of free radicals (O_2^-) at the time of ischemic reperfusion of Wistar rat hind limb was examined. Production of O_2^- in blood was determined by chemiluminescent probe (CLN method). The effect of O_2^- on functional recovery of skeletal muscle at 3 months after replantation was studied by examination of muscle wet weight and muscle contractile force. The histological condition of skeletal muscle tissue following replantation was also examined. Production of O_2^- in the 6-hour ischemia group was significantly higher than in the 3-hour ischemia group. Muscle wet weight and muscle contractile force were significantly lower in the 6-hour ischemia group than in the 3-hour ischemia group. Muscle wet weight and muscle contractile force were negatively correlated with production of O_2^-. On histological examination, the 3-hour ischemia group exhibited nearly complete recovery, but skeletal muscle disturbance was remarkable in the 6-hour ischemia group. Determination of production of O_2^- at the time of ischemic reperfusion was useful as an index for prediction of functional recovery of the skeletal muscle of replanted limbs.
