発達障害が予測される児に対し,早期からのリハビリテーションが重要といわれているが,児の状態により早期から取り組めないことがある.今回,ポジショニングとリラクゼーションを工夫することにより,リハビリテーション導入が可能となった.本研究で提示する症例は,極低出生体重児で脳性麻痔となった男児である.問題点は,1)啼泣時や不機嫌な時に異常姿勢が強く哺乳ができない.2)物音に敏感で中途覚醒が多く十分な睡眠が得られないであった.問題点1)2)に対して,ポジショニングとリラクゼーションの工夫を行い,児の反応を評価した.その結果,夜間の睡眠時間が平均5時間が7時間となった.1回の哺乳畳も100cc~180ccが160cc~170ccと安定した.哺乳時間は35分~40分が20分となった.本症例において,以下のことが確認でき,リハビリテーション導入が可能となった.1.適切な姿勢は運動や生理機能を促すうえで重要である.2.音楽は気持ちを落ち着かせ,心安らかにする.3.沐浴は筋緊張を緩和する.4.ポジショニングやリラクゼーションの工夫は,異常姿勢の緩和や生活リズムの獲得に有効である.
abstract
Early introduction of physical therapy is important for infants in who developmental disabilities are predicted. However, conditions of those infants will not allow early application of physical therapy. In this report our experience with a very low birth weight male baby with cerebral palsy is discussed. The major problems in nursing this baby were: 1) difficulty in breast feeding due to spastic posture when he was crying or anxious, 2) dyssomnia due to acoustic irritability. To resolve these problems, we adjusted the position of the baby and the environmental arrangement under which his relaxation could be expected. We evaluated the effect of the adjustments by observing his status. As a result, the average sleeping time at night increased from 5 hours to 7 hours. The amount taken at one feeding stabilized at 160-170cc, previously varying from 100-180cc. The time necessary for feeding decreased from 35-40 minutes to 20 minutes. The reduction of spastic posture and the acquisition of a regular life cycle enabled the introduction of physical therapy. This case suggests that: 1) proper position is important to promote motor function and physiological function, 2) music may calm irritability, 3) bathing may reduce muscle tonicity. Adjustment of positions and environmental arrangements are effective in reduction of spastic posture and acquisition of a regular life cycle.
